Risks, Hygiene & Legal Declarations

Please Read Carefully:

Tattooing involves inserting pigment into the skin with needles. The following risks may occur:

• Infection: Bacteria or viruses can enter through the skin if proper aftercare is not followed.

• Allergic reactions: Some clients may react to pigments or aftercare products, causing redness, itching, or swelling.

• Scarring: Improper healing, picking, or excessive irritation can lead to permanent scars.

• Fading/colour change: Over time, tattoos can fade or shift in colour due to sun exposure, skin type, and ink composition.

• Keloids: Some people form raised scar tissue (keloids), especially if prone to keloid scarring.

• Bleeding/bruising: Minor bleeding or bruising may occur during or after the procedure.

• MRI sensations: Rarely, tattoos can tingle or warm during MRI scans; inform medical staff if you feel discomfort.

• I will read Ventura Tattoo Studio’s Aftercare Instructions and agree to follow them.

• All equipment used is sterile, single-use where appropriate, and standard infection control precautions are followed.

• I am 18 years of age or older (Tattooing of Minors Act 1969). Proof of age has been uploaded.

• I consent to the tattoo design created alongside my artist on the day of the appointment being carried out today.

• I understand my personal data will be stored securely for legal and insurance purposes and will not be shared unlawfully.

I confirm I will not use any form of numbing cream during the tattooing procedure.

I confirm I have carefully read through the entire of this consent form & have filled it out accurately, I understand that any false information provided may invalidate my appointment and mean my non-refundable deposit will be lost and the tattoo not done.
